Service Application

RESIDENTIAL SERVICE APPLICATION

Establishment of Credit

The Cooperative may require you, as a residential applicant for service, to establish credit, but establishment of credit does not relieve you, from complying with the rules for prompt payment of bill. Credit history shall be applied for a reasonable period of time to a spouse or former spouse who shared the service. Credit history maintained by one will be applied equally to another without notification and without additional qualifications not required by another. An applicant shall not be required to pay a deposit if you establish satisfactory credit. Credit may be established in several ways.

Establishment of Credit for Permanent Residential Applications:

  1. Demonstrate that you have been a residential customer of a telephone company within the last two years that you are not delinquent in payment of your account with them, and that you did not have more than one occasion in which your telephone bill was paid after becoming delinquent during the last 12 consecutive months, and that you never had service disconnected for non-payment. You may obtain a letter of credit history from your previous telephone company before moving.
  2. You may furnish in writing a satisfactory guarantee from a third party to secure payment of your telephone bills. The third party guarantor must be a member of the Cooperative for at least one year, has to have established satisfactory credit of their own, and can not have been disconnected for nonpayment of their account.
  3. You may demonstrate that you are 65 years of age or older, and that you have not had an outstanding account balance with any telephone company for residential service, which accrued within the last two years. If a permanent residential applicant does not establish credit as described above, we may not require a deposit in excess of one-sixth of your estimated annual billing, including long distance carriage charges.

Initial Deposit for Permanent Residential Customers:

The Cooperative may require an initial deposit from you if you have been delinquent in paying a bill for telephone service on more than one occasion during the last twelve months of service, or if your service was disconnected for nonpayment. Such deposit may be required to be made within 10 days after the Cooperative sends you a termination notice and a request for deposit. Instead of paying an initial deposit, you may elect to pay the current bill by the due date of the bill, provided you have not done this in the previous twelve months. Instead of paying a cash deposit on the bill you may furnish a satisfactory guarantee from a third party to secure payment of bills.

Additional Deposit Requirements for Permanent Residential Customers:

During the first twelve months of service, if your actual usage is three times the estimated usage (or three times average usage of most recent three bills) and current usage exceeds $150.00 and 150% of the security held, a new deposit requirement may be calculated and an additional deposit may be required to be made within ten days after the Cooperative sends you written notice of termination and a request for additional deposit. Instead of paying an additional deposit, you may elect to pay current usage within ten days after we send you written notice of termination and a request for additional deposit.

Deposits for Weekend Resident, Temporary or Seasonal Service:

The Cooperative may require a deposit sufficient to reasonably protect it against the assumed risk for weekend residence, temporary or seasonal service. These deposits will be returned according to guidelines set out below in REQUIREMENTS FOR RETURN OF YOUR DEPOSIT.
